· contact the Family Resource Coordinator (FRC) at 304-636-2363
**You must schedule a time with the FRC office (304-636-2363) to turn in your child's application and health forms for the process to be complete. **
Items you should bring to the meeting with the FRC:
· Basic Health History Form
· Completed Pre-K application
· Certified Birth Certificate from the State
· Well Child Exam- within last 12 months
· Immunization Record
· Dental Exam Verification Form- within 6 months
· Home Language Survey
· Legal/ Custody Information- if applicable
