Here's what to bring to the appointment:

1. Your child's insurance or medical card
2. Certified birth certificate  (If you need one, the application file is below)
3. Immunization record (See the Vaccination Requirements file below)
4. Health Check Physical (including vision and hearing)
5. Dental Screening Form
6. Completed Registration Packet
and most importantly.....
7. Your child!
